Inglewood Care Centre is part of the Provincial Long Term Care Program that authorizes all admissions, transfers, and discharges, except for the private pay resident.   

ADMISSION POLICY      

All applications for admission are made to the Continuing Care Consultant at the Vancouver Coastal Health Authority (Telephone #: 604-986-7111).

Inglewood Care Centre does not have the authority to admit on its own initiative, except for private pay residents. Applicants will be determined if they qualify for subsidized care by the Vancouver Coastal Health Authority, and then placed in the next available bed. Once a person is in another facility, they do have a choice to be transferred to their preferred facility. Applicants will be given 24 hours notice of admission to the facility. Every effort is made to admit to Inglewood at a time that is convenient to both the family and resident.
   

PREPARING FOR ADMISSION

Long Term Care Assessment: The potential resident’s care level is assessed by a Long Term Care case worker, either in the community or in an acute care facility, to determine their eligibility for admission.

Facility Tours:    Tours are arranged (prior to admission) by our Social Services Coordinator, Karen Rumak  (604-913-4707), at the request of the family or potential resident. Tours usually take from 1/2 - 1 hour.

FAMILY INVOLVEMENT / RESPONSIBILITY:                                         

When a potential resident is initially placed on the admission list:, a family member usually:

bullet contacts the Social Services Coordinator for more information
bullet maintains contact with the resident’s care coordinator and the facility
bullet begins preparation for the move
bullet ensures that potential resident has had a recent medical history and chest x-ray and ensures that the resident has a family doctor on the North Shore
bullet becomes familiar with admission documents received at that time (resident agreement, trust account, degree of intervention & resident handbook)
bullet continues preparation of potential resident for the move
bullet labels all mobility aids, dentures, hearing aids, glasses and small appliances
bullet ensures clothing is appropriate (washable if being done at facility, proper shoes etc.)

"HOW DO I GET READY?" - POTENTIAL RESIDENT’S POINT OF VIEW:   

Begins to prepare personal items and belongings, such as the following:

bullet toiletries
bullet clothes (recommend the resident have at least 7 changes of underwear and 5-7 changes of outerwear available at all times)
bullet agreed-upon room furnishings and decorations
bullet aids for mobility (should be labeled)
bullet any prostheses, hearing aids, dentures or eye glasses should be labelled.
bullet notify family doctor and other professional services
of "change of address
bullet notify friends and associates of change of address
bullet decide what to do with household items that won’t be moving
bullet be aware that the living and eating environment will be communal and thus quite different from what they are used to

FACILITY RESPONSIBILITIES:

bullet Alerts potential resident and family when name nearing top of the list
bullet Arranges pertinent documents to be distributed and signed
bullet Prepares room
bullet Informs resident and family that Inglewood is not responsible for personal belongings

ADMISSION PROCESS

Social Service Coordinator:

bullet notifies family that room is ready (potential resident should be sleeping in the room within 48 hours)
bullet determines actual move date with family
bullet calls physician requesting medical history, chest x-ray. and degree of intervention if not already obtained
bullet conducts an orientation to the physical surroundings
bullet introduces Inglewood personnel
bullet arranges for photographs to be taken and clothing and personal equipment labeled
bullet reviews Resident Agreement, Trust Account and Degree of Intervention forms with resident and family and obtains signatures

Primary Nurse:

bullet completes a nursing assessment which includes such things as blood pressure, weight, history, allergies, likes and dislikes
bullet checks in all medications being used at home
bullet orders medications from our contracted pharmacy as prescribed by the resident’s physician
bullet ensures that resident and family are familiar with the resident room, dining room, personnel, etc.
bullet arranges seating in dining room

Family:

bullet assists with room arrangement
bullet attends nursing assessment meeting and orientation with resident
bullet stays with resident during moving day (lunch or dinner is complimentary for 1 - 2 family member on this day)
bullet provides emotional support
bullet arranges for newspaper delivery, if desired
bullet advises social worker if cable or telephone is desired

Laundry Label/Photo ID Charges:

bullet labelling of personal clothing (that will be laundered at Inglewood)
bullet photograph
bullet ID bracelet
bullet Room Name Plate
